MVA FORM VR-450:  Affidavit of Ownership – Moped, Motor Scooter & OffRoad Vehicle [ORV]

This form is provided by the MVA when applying for a Certificate of Title for a Scooter, Moped, Snowmobile, Dirt Bike and/or an ATV that has never previously been titled. Please bring the completed VR-450 Affidavit of Ownership with all other supporting documentation to Diversified Tag & Title Services and let us help you any way we can.

IMPORTANT: The MVA VR-337 Application for Title & Decal must be also be completed and submitted w/VR-450.
