MVA FORM VR-210: APPLICATION FOR MARYLAND PARKING PLACARDS/TAGS FOR INDIVIDUALS W/DISABILITY

This form is provided by the MVA for individuals with a disability when applying for a parking placard or license plate. The completed form may be brought into at any full service MVA office or mailed to: Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Glen Burnie, MD 21062, Attention: Disability Unit.

IMPORTANT: [1] Your physician will need to complete the disability certification portion of the application; [2] VEIP Exemption applies only to disability Tags; NOT disability Placards; [3] Each vehicle is eligible for either 2-placards or 1-placard and 1-tag.
