MVA Form VR-098: Certification for the Issuance of Dump Service License Plates

This form is provided by the MVA to certify that the vehicle is designated to haul feed or other loose or bulk materials in bulk and is self-loading by gravity or mechanical means. Please bring the completed VR-098 Certification of Dump Service Tags with all other supporting documentation to Diversified Tag & Title Services and let us help you any way we can.
