MVA Form VR-294: Tow Truck Certification

This form is provided by the MVA to certify all requirements of a Tow Truck. The owner certifies under penalties of perjury to the weight of the tow truck, that commercial insurance has been obtained and that the tow truck will only be used for towing purposes [i.e., storage, repairs and removal of abandoned vehicles, etc.].
