MVA FORM VR-461: Certified Statement/Receipt

This form is provided by the MVA because Maryland law requires customers to return their license plates to the MVA when their insurance is cancelled, when they move out of Maryland or when the license plates are no longer needed. If the customer is reporting their license plates lost or stolen, they must provide a police report or a police report number.
